Q.Certificate of Naturalization and Name Change

I had an Interview, took an Oath, got the Name Change, then got the Certificate of Naturalization (N-550) on the same day. It was 12 years ago.

I just realized that N-550 shows the new name, but does not have 2nd page (attachment for Name Change).

Some body suggested "file N-565 for New Certificate Naturalization". Here are a few questions.

1. Does New N-550 show both old and new names without 2nd page (I mean attachment) ?

2. My old N-550 has side front view of the face and left side is white, so that I signed on the left side of the picture. Picture for N-565 is front view and what about the left side of the picture ? Do I have to sign on the picture in front of INS officer or at home ?

3. When I send in N-565, do I have to send in the original old N-550 ?
