Q.HAMP if not approved, would it be possible to Short Sale?

Married in 2007
1. After the couple got married, purchased a CONDO under wife's name 5 ARM, has 1st and 2nd Loan (2 bedrooms)
Purchased: 2007
2. 2012, in the process of closing on the HOUSE under husband's name.
Needed to move to be closer to child's school, a bigger living space, and better air condition with location.
3. Plan to rent the Condo, however, the Loan value is higher than the appraised property price
Researching to see if qualified for home modification with the HAMP program
Both husband and wife work.
Tax return - file jointly
From the Loan Modification calculator, it appears that we could qualify for the HAMP program on the 1st and 2nd.
QUESTION:
For the HAMP program, since we are a married couple, do we need to include both husband and wife's information?
If the condo is not approved on the HAMP program, and we can't rent the property, would we be able to SHORT SALE on the Condo?
I am currently ill and due to this stress, my condition is worsening. Suggestions are welcomed.
