the court order or other state vital record

4번에서,If you are applying for a new document because your name was changed, you must submit the original USCIS document and a copy of the marriage certificate or a certified copy of the complete document (such as a marriage certificate or certified copy of a court order, amended birth certificate, or amended passport) showing the name change. 라고 설명하고 있습니다.

그래서, 말씀 하신데로, 시민권증서 카피, 법원판결문원본,N-565 신청서, Fee $345 제출하시면 됩니다.
