“If you live in a foreign country when you turn 65 and first qualify for Medicare, you can sign up by requesting enrollment forms from the United States Embassy or Consulate in the country where you currently live.

The Social Security Administration (SSA) should also send you a letter explaining that you have entered your Initial Enrollment Period (IEP), along with a form to request an enrollment packet.

* Be sure to keep copies of any forms that you complete. If possible, it is best to send paperwork via certified mail to ensure their delivery.

If you were collecting Social Security benefits when you became Medicare-eligible, you may have been automatically enrolled in Part A.

You're over 65, currently getting Social Security retirement benefits and Part A, and you didn't take Part B when you were first eligible (IEP).
You may only apply for Part B from January 1–March 31 each year; General Enrollment Period (GEP).
And, you may have to pay a late enrollment penalty for as long as you have Part B. “

Note that this information only applies if you are living abroad; it does not apply if you are simply taking a trip. If you have specific questions, contact the Social Security Administration (800-772-1213) or the Consulate or Embassy in your country of residence before making any final decisions about Medicare enrollment.

SOCIAL SECURITY ADMIN.

"If you have a Medicare Advantage ( Medicare Part C) or Medicare Part D plan before you move abroad, you should DISENROLL and stop paying these premiums when you move because these plans require that you live in their service area in order to be enrolled.

Medicare Advantage plans (also known as Medicare Part C) are an alternative to Original Medicare, Part A and Part B. Instead of having Medicare benefits administered through the government-run program,

beneficiaries can choose to get their coverage through a Medicare Advantage plan, available through private insurance companies that contract with Medicare.

Since most people pay no premium for Part A coverage, it is usually best to keep Part A, even if you are moving abroad, because it is free.

To have Part B coverage, you must pay the monthly Part B premium. You may want to keep Part B if you plan to move back to the U.S. in the future or visit frequently.

This is because paying the premium to keep Part B when abroad will ensure that Medicare will cover your care whenever you travel to the U.S., and that you will not face premium penalties or gaps in coverage.

If you fail to pay for Part B while abroad, when you move back to the U.S. you may go months without health coverage. This is because you may have to wait until the General Enrollment Period (GEP), which runs January 1 through March 31 each year, with coverage starting July 1.

The general rules of Medicare eligibility, enrollment periods and penalty rules apply regardless if a person (U.S. citizen) resides in the United States or a foreign land.

“You're over 65, currently getting Social Security retirement benefits and Part A, and you didn't take Part B when you were first eligible (IEP).
You may only apply for Part B from January 1–March 31 each year; General Enrollment Period (GEP).
And, you may have to pay a late enrollment penalty for as long as you have Part B. “

“If you're over 65, and are eligible for Social Security benefits, you may file an application for monthly benefits and Part A. You'll have to file for Part B from January 1–March 31. And, you may have to pay a late enrollment penalty.”

The law says that a 10 percent penalty is imposed "for each full 12-month period" that people delay enrolling in Part B when eligible to do so (unless they have group health insurance from their own or their spouse's current employer). If the delay is less than 12 full months, the penalty should not apply."

*Note that since 2012, the late sign-up penalty for Part B can be waived. For example, if you were a US citizen living outside the US at the time you turned 65 and DIDN'T qualify for Social Security benefits at that time, you may sign up for Part B at a later date without having to pay the late sign-up penalty if and when you return to the US to live there.

Some people don’t qualify for Medicare Part A benefits without paying monthly premiums for them because they or their spouses haven’t contributed enough in payroll taxes at work. In this situation, you cannot sign up for Part A or Part B outside the United States.

Therefore, in this specific circumstance, you can delay Medicare enrollment until your return, without being subject to late penalties — regardless of how long you lived outside the U.S. or how many years have passed since you turned 65. Your special enrollment period begins during the month of your return as a U.S. resident and expires at the end of the third month following. Coverage begins on the first day of the month after you enroll.

You can usually delay signing up for Part B beyond age 65 without risking late penalties if:
(a) You are covered by a group health insurance plan from your employer as’ the primary insurer’ or union and you are still working, or
(b) You are covered by your spouse's group insurance and your spouse is still working."
